Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate’s experience in the exact or comparable position and additional preferred qualifications will be considered.
Purpose of Position: The Licensed Practical Nurse will act as a key member of the Nursing Department, providing support and assistance and will ensure to adhere to policies and procedures of the agency, the states professional standards and regulatory entities including DOH & OMIG guidelines for a Licensed Home Care Service Agency (LHCSA).
Schedule: Monday – Friday (Day Shift)
Salary: $65k per year
Roles, Responsibilities and Essential Duties:
- Review medications with patients
- Educate patients on health maintenance and disease prevention
- Conduct wellness checks of their physical and mental state
- Update medical orders and verify info contained therein
- Review Plan of Care with Home Health Aides
- Assist in the maintenance of the Plan of Care in our Electronic Health Record System
- Conduct reviews of clinical charts for compliance
- Complete HHA annual paraprofessional evaluations
- Additional related duties as assigned
Required Skills/Qualifications:
- NYS Licensed Practical Nurse
- Basic Cardiac Life Support certificate required
- LPN Malpractice Insurance strongly recommended
- Bilingual Spanish language skills required
Skills, Knowledge & Abilities:
- Excellent organizational & time-management skills
- Basic computer skills; knowledge of data base entry and email (i.e. Outlook)
- Skilled in interpersonal relationships
- Excellent documentation skills, ability to meet deadlines
- Ability to carry out instructions and maintain records
- Ability to assume delegated authority and accountability for care of assigned patients
- Ability to function independently with minimal supervision
